ANY NEW ENROLLMENTS OR CHANGES WILL BE EFFECTIVE FOR THE PLAN YEAR BEGINNING JANUARY 1ST 2024.
Failure to enroll by the deadline will be considered a waiver of voluntary benefits. The next time to enroll in voluntary benefits coverage will be during the next applicable open enrollment period which is Nov. 1 - Nov.30 2024 for benefit year beginning 1-1-2025, unless there is a qualifying event (Contact HR for a list of qualifying events).

Employees who are currently not signed up for benefits and do not wish to enter, may waive all coverage by clicking the button and completing the form below.

Employees who are currently signed up for any voluntary benefits and want to drop/change coverage, must contact the benefits department to complete the appropriate paperwork
