Thank you for doing your part to protect yourself and those you love by getting the COVID vaccine parts 1 & 2.
Choices will give every employee who receives the shots by June 30th, 2021 $400.
To participate you must:
- Be an active CICL employee
- Provide Proof (photo or photocopy of card paperwork and completed form attached)
- $400 will be paid upon our receiving proof (paid by last day of the month beginning February)
- The Covid payout will be a non-taxable item added to payroll prior to the end of the month
