Qualified Workers COVID-19 Vaccinations

Public Health - Dayton & Montgomery County will be hosting a vaccination clinic specifically for individuals who qualify for COVID-19 vaccinations based on their occupation.

The Vaccination Clinic is by prior appointment only, no walk-up service will be provided.
(Please note: You must show work ID or other documentation to show proof of inclusion in this phase.  
If you do not have organization identification, please bring a recent pay stub or a letter from your employer as proof of employment.
Identification will be checked at the door.  You will not be able to receive a vaccine without it.)

All previously-scheduled appointment times will be honored. Please remember: All people receiving their second dose must bring their vaccine card and another signed consent form: http://bit.ly/ZiksPharmacyConsent.
